Position Description

Spectrum Healthcare Resources has a potential need for a Registered Nurse – Case Manager at David Grant Medical Center, Travis Air Force Base, CA. The Nurse will provide case management services within the Integrated Disability Evaluation System. This is a full-time, on-site position, working Monday through Friday, 7:30am-4:30pm. Requirements
  • Associate Degree in Nursing from an accredited program.
  • Current, full, active, and unrestricted RN license.
  • At least 3 years of experience within the last 4 years working as a registered nurse case manager.
  • Current Basic Life Support (BLS) certification.
  • Possess one of the following certifications:
    • Certified Case Manager (CCM)
    • Certified Disability Management Specialist (CDMS)
    • Certified Rehabilitation Registered Nurse (CRRN)
    • Certified Occupational Health Nurse (COHN) or COHN-S
    • Advanced Certification in Continuity of Care (ACCC)
    • Certified Rehabilitation Counselor (CRC)
    • Nurse Case Manager (RN-NCM)
    • Care Manager Certified (CMC)
Duties
  • Participate in all phases of the Case Management Program (CMP) to ensure established CM standards of care are met.
  • Provide nursing expertise in assessment, planning, implementation, coordination, and monitoring of case management processes.
  • Develop treatment plans that ensure continuity of care and optimal wellness outcomes.
  • Liaise with community agencies and organizations to ensure seamless care transitions.
  • Educate patients and caregivers on their care plans.
  • Facilitate interdisciplinary team meetings and participate in discharge planning.
  • Ensure accurate data collection and documentation.
  • Conduct peer reviews and quality reviews as required.
  • Travel outside the local MTF may be necessary (reimbursable as applicable).
  • Strong organizational, communication, and problem-solving skills required.
  • Proficiency with Microsoft Office applications and familiarity with military healthcare systems.
