RN Case Manager
StartDate: 1/12/2026 Pay Rate: $1800.00 - $2700.00
POSITION SUMMARY – RN Care Coordinator
POSITION DUTIES – Discharge Planning / Care Coordination / Continuum of Care / Patient Focus and Care Plans / Resource Utilization
MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; COVID Vaccine; Crisis Protocol; 3+ years nursing experience in an acute hospital setting; Prior Care Coordination experience in a clinical setting
PEFERRED QUALIFICATIONS – Previous Care Coordinator experience in a Dignity/CommonSpirit facility
LENGTH OF ASSIGNMENT – 8 weeks
SHIFT / HOURS PER WEEK – 10 hr Days, EOW
SYSTEMS – Cerner
START DATE – 1/6/2026
